Dr. Conrad:
“During your rhinoplasty consultation, I will perform a detailed exam of the nasal structure externally and internally. I note the shape, size, and skin condition of the nose. I also observe carefully how the nose relates to the patient’s overall facial features, head shape, and profile.
“I will evaluate the nasal structure and assess the patient’s breathing. I will examine the nose to determine if there are any abnormalities such as allergic condition or narrowing of the nasal passages. During full consultation photographs will be obtained for preoperative planning. I will prepare drawings to indicate my surgical intention. This approach allows patients to determine if the surgeon has artistic ability to design the nose that will coincide with their desire. Computer imaging is also available for those interested but it does not reveal the surgeon’s talent and judgment to the same extent.”

Dr. Conrad:
“We all know what a dramatic esthetic and psychological effect nasal surgery can have. It is necessary that the patients are at the age when the nose has fully developed, usually 14-15 years and they have a mature attitude and understanding of the importance of their decision.
Dr. Conrad:
“Primary rhinoplasty is surgery performed on a nose that’s never been operated on before. Rhinoplasty can vary as to the extent of the change that is necessary. Even when the procedure seems straightforward, the surgery requires meticulous skill for the best results. The simplest rhinoplasty done by an inexperienced surgeon can lead to poor results.”
Dr. Conrad:
“Reduction rhinoplasty is surgery to reduce the size and enhance the shape of the nose. Usually, in these procedures, the rhinoplasty is achieved by reconfiguring the existing bone and cartilage and at the same time if necessary the inside anatomy of the nose may be perfected to preserve or improve nasal breathing passages. Most reduction rhinoplasty does not require foreign material or grafts from other parts of the body.”
Dr. Conrad:
“Augmentation rhinoplasty is surgery to build up the nasal bridge and possibly add strength to the nasal tip. In these surgeries, implants or grafts may be needed, often in patients of African or Asian descent, to achieve the desired result.”
Dr. Conrad:
“This is a form of rhinoplasty in which the fundamental nasal structure requires reconstruction. In these cases, we can use cartilage grafts from a patient’s nose, ear, or from another part of the body, for example a rib cartilage. I prefer to use a patient’s own tissues if possible, but in some augmentation rhinoplasty procedures, I may consider the use of medical grade implant materials. These materials are surgically dependable and used widely all over the world.
“For some patients using implant material is a good option, especially when a patient has limited tissue to use as a result of prior rhinoplasty surgery, or if the patient does not wish to remove cartilage from their body. A detailed discussion regarding advantages and disadvantages of the available choices is always conducted with patients.
Dr. Conrad:
“There are, of course patients for whom the primary goal of surgery is to correct breathing problems. Some of these patients do choose to have cosmetic surgery at the same time. It is most important, therefore, that the patient and I develop a clearly-understood concept of the reason for the changes to the nose.
“I make certain that my patients know there could be various restrictions as to what can be accomplished in cosmetic nasal plastic surgery. This can include the patient’s ethnicity and skin type. Simply put, in every case of rhinoplasty, issues of esthetics and function are equally considered. This means that each patient’s case is carefully reasoned and that one’s unique facial attributes are carefully assessed.”
